We ask that you complete the virtual intake form below as the first steps to initiate your DUOver Experience. A representative from DUO will follow up within 48hrs with the proper steps and invoice to move on to the next steps of your DUOver Experience. Invoice must be paid within 24hrs in order to move on to phase two of your DUOver Experience. We ask that you provide us with as much detail as possible so that we are able to provide you with the best DUOver Experience and adequate DUOgear. Please note that this form must be filled out and your DUOver Experience MUST be initiated 16 business days prior to event date in order to protect the quality of our craft. If event date is less than 16 business days away but less than 9 business days away there will be a expedited fee attached included when invoiced. If your event is 9 business days or less away unfortunately at this time DUO will not be able to accommodate!
